Becoming a patient at The Root Cause Wellness Center is an easy process. Everyone receives a free consultation. That is a one hour discussion about your health. If we feel that you are a good candidate, you will take home a packet of forms to fill out and return. Part of finding the root cause is being an investigator.

 

INITIAL MEDICAL VISIT

 

Please allow approximately 2 hours for your initial visit. This will include an in-depth consultation with Dr. Razak and LeAnn, our holistic health coach. Diagnostic laboratory testing and nutritional therapy will be integral components of your treatment plan. Test results are used to support us in designing your personal health care program, as well as uncover the root cause of symptoms you may be experiencing. We will review all the laboratory finding with you and review the nutritional supplements that may be recommended and help you select the highest quality products.

 

FOLLOW UP MEDICAL VISITS

 

At your first follow up with the team, you will receive detailed laboratory test results and explained the findings. This visit will last approximately 45 minutes. Depending up your unique health conditions, your treatment plan will vary. Based up on your needs some patients will require follow visit every few months where as other may need to be seen less frequently. Other patients may require follow up visits with the health coach only.

 

INSURANCE COVERAGE

 

Our office does not bill insurance. We can provide you with a coded receipt for services, which you may submit to your insurance carrier for reimbursement, and depends upon your insurance provider.

 

Services associated with our clinic including physician visits, nutritional counseling, supplements and laboratory testing can be submitted under your Health Savings Account or healthcare Reimbursement Account.

 

FEES & PAYMENT

 

The Root Cause Wellness Center operates as a self pay clinic. We accept cash, checks or credit cards for services rendered. Payment is expected at time of service.

To cancel an appointment, please call us at least 48 hours in advance. You may leave us a message if you are unable to reach us directly.
